WAIVER AND RELEASE OF LIABILITY

The coaches, trainers, and assistants of the Roaring Fork Women’s Triathlon Team, Tri Valley Training, the Community Center of Glenwood Springs, and the Hot Springs Pool and Athletic Club, urge you and all members to obtain a physical examination from a doctor before using any exercise equipment or participating in any class.

All exercises, including the use of weights and use of any and all equipment, swim aids, and all apparatus designed for exercising shall be at the group member’s sole risk. Member understands that the agreement to use a selection of exercise programs, methods, and types of equipment shall be member’s entire responsibility, and the coaches, trainers, assistants, Tri Valley Training, the Community Center of Glenwood Springs, and the Hot Springs Pool and Athletic Club, shall not be liable to member for any claims, demands, injuries, damages, or actions arising due to injury to member’s person at facilities, or premises of the facility or training terrain.

Member hereby holds the Roaring Fork Women’s Triathlon Team, it’s coaches, trainers, and assistants, Tri Valley Training, and the Community Center of Glenwood Springs, and the Hot Springs Pool and Athletic Club their officers, owners, agents, and employees harmless from all claims which may be brought against them by member or on member’s behalf for any such injuries or claims.

I certify and represent that my physical condition is adequate to participate safely in this training group and acknowledge that the coaches, trainers, assistants, and above facilities have no obligation to provide medical care and have not undertaken any responsibility to do so. In the event that I receive medical care as a result of a medical emergency, I hereby consent to such care and fully release the person(s) providing such care from any and all liability, whether resulting from negligence or otherwise.

I assume all risks associated with participation in this training group including but not limited to cardiac arrest, falls, injuries, contact with other participants, vehicles or animals, weather conditions, and hazards that may be posed by traffic, roads, trails, or spectators, and accept that I am responsible for my own safety.
